Custom Reports

Do you need your report to do a little more?  Do you need a report that prints your families' information in a unique way?  We can create it for you!  We have been able to assist many organizations' reporting needs by writing custom reports or by modifying existing reports.

In order to process your request, please read the PDS Custom Report Guidelines (PDF, 20K) then fax the completed PDS Request for Custom Report form (PDF, 28K) to 602-567-7783.

The current wait time for custom reports is three months.
